Liability Waiver & Release — I understand that participation in Wulfspar Roblox Game Creator Camp involves physical activity and use of computer equipment. I agree to release Wulfspar Nexus, its owners, staff, and volunteers from any liability for injury, illness, loss, or damage arising from my child's participation in camp activities, except in cases of gross negligence or willful misconduct. * Medical Treatment Authorization — In the event of a medical emergency and if I cannot be reached, I authorize Wulfspar Nexus staff to seek emergency medical treatment for my child. I agree to be responsible for any medical costs incurred. * Photo & Video Release — I grant Wulfspar Nexus permission to photograph and/or video record my child during camp activities for use in marketing, social media, and promotional materials. I understand no personal identifying information will be shared without additional consent. * Code of Conduct — I understand that my child is expected to treat staff, instructors, and fellow campers with respect. Bullying, harassment, or destruction of equipment may result in removal from camp without refund. * Refund & Cancellation Policy — I understand that deposits are non-refundable. Weekly camp passes may be refunded or credited if cancelled at least 5 business days before the camp start date. No refunds will be issued for cancellations made less than 5 business days before camp begins or for no-shows. * Accuracy Confirmation — I confirm that all information provided in this form is accurate and complete to the best of my knowledge. I am the parent or legal guardian of the camper named above and am authorized to sign on their behalf. *
